AMC Business Card Order Form
AMC staff members need to login to access the form below.
Staff are asked to complete the online business card request form if they wish to receive a set of business cards. Please be advised that requests for business cards are processed once approval is granted from your National Centre director or line manager.
Business card format notes:
Name
<first name> <surname>
Where a name takes its origin from another country, naming conventions can be used consistent with the appropriate thematic structure
Eg. John Smith
Eg. Abdul Rahman bin Mohamad Sidek
Eg. Wong Lai Chai
Qualifications
Qualification abbreviations stipulated by the degree granting institution
Position title
Eg. Lecturer, Maritime Training Ocean
Eg. Senior Lecturer, Department of Maritime Engineering
Eg. Lecturer, Department of Post Harvest
Special requests
If you would like to include additional information on your business card, please complete the ‘special request' field at the end of the online form. Staff holding Tickets or a professional status (eg. CPA) should include details in the ‘special requests' field.
